Consultation – can you help us please?

We have set up a consultation that runs from now until 10 August to find out your views about:
- The core data to be collected at the notification of a death, at an initial 'light touch' local review, and in a full multidisciplinary review of the death of a person with learning disabilities (LD).
- The criteria for which deaths are would require an initial ‘light touch’ local review only, and which will require full multiagency review.
- The definition of key terms used in the Programme.
- The standards against which ‘best practice’ will be measured.

Please could you help us by contributing your views to the consultation? We appreciate your help with this and would like you to contribute as much or as little as you can.
Please don't feel that you have to respond to every section or question.
You need to fill in the first page for us to let us know about yourself, but then you can choose which sections to complete. You might, for example, only be interested or able to comment on the definition of terms we will be using (Page 10) so if that is the case just fill in that part. Alternatively, you might want to go through the whole of the consultation document and respond more fully. You can complete the document in stages, returning to it at a later stage if that is more convenient, as long as it is submitted by the closing date of 9 am on 10 August.
