Description including Unit Aims
Senior Medicine and Surgery: 12 week unit
- Senior medicine to include Neurology teaching
- Senior surgery + first part of the Surgical Skills course
- Palliative Care and Oncology core teaching + tutorials + clinical placements
- Acute Care - week of Emergency Medicine/acute admission unit work
- Radiology
Intended Learning Outcomes
- To be able to demonstrate understanding and knowledge of the symptoms and signs of common medical and surgical conditions.
- To be able to take a full and accurate clinical history and to be able to present this succinctly this to others.
- To be able to undertake a full and comprehensive clinical examination within an appropriate time frame.
- To be able to undertake intimate examinations (breast and rectal) appropriately and effectively.
- To be able to formulate a differential diagnosis.
- To be able to employ appropriate and economical investigations to confirm a diagnosis.
- To be able to devise an appropriate initial plan of management.
- To have an understanding of and to be able to demonstrate principles of safe prescribing, including analgesia.
- To be able to recognise, assess and manage the acutely ill or collapsed patient, including immediate life support.
- Understand and be able to demonstrate recognition of medical priorities.
- To have an understanding of and to demonstrate good practice in medical record keeping.
- To have an understanding of and to demonstrate safe use of appropriate medical devices (eg: sphygmomanometer; oxygen saturation monitor; I.V. infusion set; hand-held Doppler; urinary catheter).
- To be able to demonstrate performance of appropriate diagnostic and therapeutic practical procedures, with respect to medicine and surgery, as listed in the GMC’s Outcomes for Graduates – Practical skills and procedures (2020).
- To be able to handle surgical instruments and sharps safely and appropriately.
- To be able to demonstrate basic suturing skills.
- To develop and be able demonstrate professionalism in the workplace (Polite, respectful and non-judgemental with patients, relatives and staff. Respects patients' privacy and dignity. Maintains patient confidentiality).
- To develop and demonstrate appropriate communication skills with patients and relatives including breaking bad news.
- To have a comprehension of the law, ethics and guidance surrounding "Do not attempt resuscitation orders".
- To have an awareness of the ethics, principles and legalities of valid consent.
- To develop awareness of assessment and planning for home discharge and management of chronic disease.
- To have an understanding of and be able to demonstrate principles of infection control (Hand washing; aseptic techniques; personal protection; sharps disposal and injuries; use of antibiotics; development of resistance; blood borne viruses.
- To have a basic understanding of nutritional care.
Teaching Information
Palliative Care and Oncology core teaching
Within Academies, teaching is centred on patients, such as by bedside teaching and small group discussion of cases. There is classroom-based teaching on prescribing, neurology and radiology. There is also surgical skills training in the clinical skills training centres.
Assessment Information
Finals will be comprised of 11 elements:
1. a best-of-5 Applied Knowledge Test (AKT), consisting of one written paper and the Bristol Clinical Data Examination (BCDE)
2. the Prescribing Safety Assessment (PSA)
3. a clerking portfolio, completed during Senior Medicine and Surgery (SMS) and marked by staff at the academy where the student studied this unit
4. 16 Entrustable Professional Activities (EPAs) signed-off in the Year 5 workbook
5. the Consultation and Procedural Skills (CaPS) logbook
6. a Team Assessment of Behaviour (TAB)
7. an elective plan marked by the student’s elective supervisor
8. satisfactory completion of the elective
9. satisfactory completion of Senior Medicine and Surgery
10. satisfactory completion of Preparing for Professional Practice
11. satisfactory engagement with the Professional Mentorship Scheme
